Same way you did the first one. Sign up using the name you want to use as your "real" name.

You can create an alternate email address that will go to the same mailbox as your primary email ID. Just go to this page. http://edit.yahoo.com/config/list_alias .
The other option is to sign out and at sign in create a second Yahoo account.
